INTRODUCTION 21tioned above, Qing dynasty gu worm school practitionershave identified certain Chinese medicinals that are especiallyeffective for treating such conditions. The previous tableshows the most commonly used anti-gu medicinals accordingto modern Chinese medicinal categorization. Therefore, whenever treating complex chronic diseases associatedwith intestinal dysbiosis or polysystemic chronic candidiasis,we recommend choosing at least some of the medicinalsin one’s formula based on their specific anti-gu functionsand abilities.HERXHEIMER REACTIONSHerxheimer reactions refer to die-off reactions when largeyeast and fungi populations are suddenly killed off. 25 Manyof the anti-gu medicinals described above are described byWestern pharmacologists as having pronounced fungicidalability. Therefore, it is not uncommon to provoke such dieoffreactions when first administering a yin fire/anti-gubased formula to a patient with marked intestinal dysbiosis. In that case, there will be nausea and vomiting, headache, possible fever, possible body aches, and a general sense ofmalaise. These symptoms are caused by the toxicity of thebreakdown products produced by the dying yeast and fungiand the body’s attempt to purge these toxins by any meanspossible. Although such Herxheimer reactions can beintense, they are usually of short duration, typically lasting6-12 hours and not more than a maximum of 36 hours. Ifone does provoke a Herxheimer reaction, see suggestionsbelow for what to do.1. Stay calm (both patient and practitioner)2. Drink plenty of water3. Soak in an Epsom salts bath4. Purge the bowels using Magnesium citrate purchasedfrom the local pharmacy or add Mirabilitum(Mang Xiao) and/or Radix Et Rhizoma Rhei (DaHuang) to the patient’s formula5. Keep taking the Chinese medicinalsAlthough we may think that a Herxheimer reaction is a signthat we have prescribed the wrong Chinese medicinal formula,in this particular case, that is not so. The formula is correct. It is just that the patient’s yeast and fungi populations arevery large. If we stop administering the Chinese medicinalformula for fear that it is an erroneous formula which is causingthese unwanted side effects, then we will stop killing theseyeast and fungi. Although all side effects are normally considereda sign of inaccurate, erroneous treatment in Chinesemedicine, this is one instance where a “healing crisis” is trulywarranted. We will know that these symptoms are aHerxheimer reaction due to the limited length of time theypersist and by the fact that all the patient’s signs and symptomstake a great leap forward in terms of improvement asthese flu-like symptoms abate. If these reactions continuebeyond 36 hours or are not followed by across-the-boardimprovement in the patient’s condition, then these symptomswere not a Herxheimer reaction. The formula was indeedwrong, and it should be stopped while we re-evaluate thepatient’s pattern discrimination from top to bottom. When treating either a yin fire or gu worm condition, it isimperative that the patient eat a yeast-free, hypoallergenic, clear bland diet. For more information on these aspects ofChinese dietary therapy, the reader should see Bob Flaws’sThe Tao of Healthy Eating (Blue Poppy Press). Trying to treatthese kinds of conditions without proper dietary therapy islike bailing water from a sinking boat without plugging theleak. Such a clear bland diet should be adhered to strictly forthe first 3-6 months at the very least and moderately strictlyfor a year to 18 months after that. Further, the patientshould be advised that they should not completely revert totheir previous diet or their condition will surely relapse. It isour experience that, in clinical practice, most relapses ofchronic, remittent diseases are due to dietary indiscretions. For further information on yin fire and gu worms, please seeBob Flaws’s freely downloadable articles at www.bluepoppy.com.LIVER DEPRESSIONBecause the primary immediate cause of liver depression isunfulfilled desires and everyone with a chronic disease hasunfulfilled desires, we agree with the famous Chinese geriatricspecialist Yan De-xin that, even if liver depression didnot cause the disease, liver depression does complicate everychronic disease. 26 In addition, in Chinese gynecology thereis the saying, “In adults, blame the liver.” This is because alladults living in civilized societies must have unfulfilleddesires. In fact, delayed gratification or non-gratification ofdesires is most peoples’ definition of being an adult. Therefore, it is very important to understand all the causesof liver depression. Some of these have been touched onabove. However, since this is such an important aspect ofpattern discrimination in real-life patients, we believe itbears reiterating. While unfulfilled desires are the primary immediate cause ofliver depression, there are a number of indirect causes of liverdepression. First of all, anger may damage the liver. If angeris unexpressed, that itself is a very powerful unfulfilled desire. However, if anger is expressed, that results in over-coursingand over-discharging. Since any activity which is extreme, being too much or too little, may damage its correspondingChinese organ, the secondary result of this over-coursing isthat the liver loses its normal control over-coursing and discharge.Hence the liver becomes depressed and the qibecomes stagnant.
