Stage 4 = Severe dissability; still able to walk or stand unassisted.<br />

Stage 5 = Wheelchair bound or bedridd<strong>en</strong> unless ai<strong>de</strong>d.<br />

VI. Scoring of motor fluctuations<br />

A. No motor fluctuations or dyskinesias.<br />

B. Dyskinesias and/or akinetic episo<strong>de</strong>s (early morning, <strong>en</strong>d-of-dose, or other)<br />

and/or freezing gait that do not affect daily activities.<br />
