Concentratia pota<strong>si</strong>ului seric [K + ] este determinata in<br />

primul rand de:<br />

pH-ul fluidului extracelular<br />

marimea stocului de K + intracelular.<br />

In acidoza extracelulara, o proportie mare a excesului de<br />

protoni este tamponat intracelular: are loc un schimb de K +<br />

intracelular pentru H + extracelular. Aceasta deplasare a K +<br />

poate produce hiperkalemie periculoasa.<br />

In alcaloza (fluidul are pH crescut), deplasarea are loc in<br />

sens opus: K + se muta in celule in schimbul protonilor.<br />

In absenta unei tuburari acido-bazice, K + seric reflecta<br />

stocul total corporal de pota<strong>si</strong>u. La pierderi externe exce<strong>si</strong>ve de<br />

pota<strong>si</strong>u, concentratia serica [K + ] se prabuseste: o pierdere de<br />

10% a K + total corporal, coboara [K + ] serica de la 4 la 3<br />

mEchiv. / L la un pH normal (7,4). Este o scadere de 25%.<br />

Clinic, controlarea balantei de pota<strong>si</strong>u se poate face prin<br />

masurarea admi<strong>si</strong>ei de pota<strong>si</strong>u <strong>si</strong> a excretiei urinare de pota<strong>si</strong>u.<br />
