(a) This form, duly completed by the apphcant and with the certificate in paragraph 21 duly
signed by a Govemment Medical Officer, should be taken personally by the appUcant not less than
one month before the date on whieh he wishes to begin work as a teacher to the Commissioner,
who \vi\\ forward it to the Director of Education with such comments as he may think fit on the
applicant and his suitabiHty for registration and/or Ucensing as a Secondary School Teacher.
(6) The following documents must be attached to this form by the applicant and will be retumed
to him in due course :
