Note.—(a) If the Government Medical Officer who examines the appUcant is unwilUng for any reason to
sign the above certificate, he should attach a letter addressed to the Director of Education
explaining the circumstances ; he may if he prefers state the circumstances in a confidential
letter to be sent direct to the Director of Education.
(b) AppUcants are not entitled to free medical examination by Govemment Medical Officers for
the purpose of obtaining this certificate.
