Pennsylvania Information Management System Pennsylvania Department of Education Special Education Domain All Pennsylvania LEAs must submit the following templates as part of the 2008-2009 Special Education Data Collection: • Special Education Snapshot • Special Education Services Fact • Special Education Events Timeline for Submission • The two snapshot dates for the 2010/2011 school year (12/1 and 7/15) The following fields submitted in the Student Snapshot templates will also be used for Special Education Reporting: • Last Name Long • First Name Long • Middle Initial • Birth Date • Gender Code • Ethnic Code Short • LEP Participation • Current Grade level • District Code of Residence • Location Code of Residence • Race or Ethnicity Subgroup Code Responsibility of reporting Special Education Students • The District of Residence will be responsible for reporting the templates in the Special Education Domain. • EXCEPTION: Institutionalized (1306) students will be reported by the host district of the facility where the student is receiving services. (Rehabilitation Center) NOTE: The “Field Name” column within each template represents the IEP Field Name. In some instances, the IEP field name did not correspond with the “eScholar Field Name”. The actual eScholar Field Name has been populated in the “eScholar field name” column of the template. It is necessary to know the actual eScholar field name in order to resolve any issues during the data submission process. Version 1 Page 237 Published 7/21/2010

Special Education Snapshot (52 Fields) Template Number: 0590 Target Table: SPECIAL_ED_SNAP Template Description This template describes the primary details of students' participation in special education as of 12/1 of the current school year. Only students in school age programs K4 – 12 who have a valid IEP on 12/1 should be included. NOTE: The 6/30/2011 Special Education Snapshot should include any students who were missed during the 12/1 submission as well as any students who may have exited as of 6/30/2011. Grain: One record per District Code / School Year Date / Student ID / Snapshot Date Please note that only the Fields highlighted will be collected for Pennsylvania Department of Education.
