Template Description
This template records the details of special education transition events as identified in Section 111 of special education students’ IEPs as of 12/1 of
the current school year.
• There must be a Transition event record for students 14 or older with a valid IEP as of 12/1.
Evaluations:
The Evaluation information collected in this template will be used to recreate Table 8A from the PennData 2010/2011 Resource Guide.
Transitions:
This is the coordinated set of activities designed within a goal-oriented process that promotes movement from school to post school.
Please enter 1 record for each goal identified in Section III of the IEP.
Grain: One record per District Code / School Year Date / Student ID / Event Type Code / Event Date
