weaknesses and, eventually, the design of policies to exploit most effectively the<br />

strengths and remedy the weaknesses. The SWOT analysis—the analysis of<br />

strengths, weaknesses, opportunities, and thre<strong>at</strong>s—is a diagnostic and plann<strong>in</strong>g<br />

tool commonly used <strong>in</strong> the priv<strong>at</strong>e sector th<strong>at</strong> could also be used as a logical<br />

framework for our purposes.<br />

The example of a SWOT analysis for Morocco (box 4.8) reveals the country’s<br />

current competitive advantages and shortages <strong>in</strong> the health sector. The analysis<br />

covers strengths and weaknesses, which <strong>in</strong>clude the price competitiveness,<br />

availability, quality, and reput<strong>at</strong>ion of health personnel (doctors and nurses),<br />

geographical and cultural factors (such as language), respect for <strong>in</strong>tern<strong>at</strong>ional<br />

management standards and standards of hygiene (for example, cl<strong>in</strong>ics certified<br />

by the <strong>Intern<strong>at</strong>ional</strong> Organiz<strong>at</strong>ion for Standardiz<strong>at</strong>ion), a tradition of exports,<br />

the organiz<strong>at</strong>ion of the sector (public versus priv<strong>at</strong>e, the critical mass of cl<strong>in</strong>ics),<br />

and so on.<br />

In a second step <strong>in</strong> determ<strong>in</strong><strong>in</strong>g whether a country could enter and compete on<br />

the world health services market, the SWOT analysis focuses on opportunities and<br />

thre<strong>at</strong>s, th<strong>at</strong> is, external factors affect<strong>in</strong>g the future of the market. From a trade<br />

perspective, this analysis is important because countries want to diversify <strong>in</strong><br />

sectors where <strong>in</strong>tern<strong>at</strong>ional demand is grow<strong>in</strong>g and not decl<strong>in</strong><strong>in</strong>g. Also, the parallel<br />

read<strong>in</strong>g of strengths and opportunities helps <strong>in</strong> identify<strong>in</strong>g the trade niches on<br />

which countries could concentr<strong>at</strong>e their efforts. Among the opportunities identified<br />

<strong>in</strong> the Maghreb are the ag<strong>in</strong>g of the European popul<strong>at</strong>ion (and the parallel<br />

<strong>in</strong>crease <strong>in</strong> the demand for care, retirement facilities, accompaniment personnel);<br />

the health supply shortages <strong>in</strong> Europe and <strong>in</strong> neighbor<strong>in</strong>g develop<strong>in</strong>g countries<br />

(wait<strong>in</strong>g lists for certa<strong>in</strong> oper<strong>at</strong>ions, skill shortages <strong>in</strong> Africa); the boom <strong>in</strong><br />

cosmetic surgery, thalassotherapy, and health tourism generally; and the gaps <strong>in</strong><br />

European health <strong>in</strong>surance schemes (and the deterior<strong>at</strong>ion <strong>in</strong> the levels of reimbursement,<br />

particularly <strong>in</strong> dentistry). The trade niches identified <strong>in</strong>clude the<br />

dental and prosthesis market, the French-speak<strong>in</strong>g comfort surgery and wellness<br />

market, retirement residential programs, and so on.<br />

Not all strengths and weaknesses have the same weight <strong>in</strong> trigger<strong>in</strong>g trade.<br />

Quality rema<strong>in</strong>s the ma<strong>in</strong> driver of trade <strong>in</strong> the health sector. The entry costs <strong>in</strong><br />

the health services market are therefore high <strong>in</strong> terms of educ<strong>at</strong>ion, tra<strong>in</strong><strong>in</strong>g, and<br />

equipment. Some countries, such as Morocco, benefit from traditionally<br />

well-tra<strong>in</strong>ed medical personnel. One cannot acquire a qualified labor force and,<br />

even less, a reput<strong>at</strong>ion overnight. Some countries therefore resort to foreign labor<br />

<strong>in</strong> the short run to supplement their domestic resources. In Malaysia, for example,<br />

the government has <strong>in</strong>creased the allowed stay under a medical visa from<br />

30 days to 6 months. It is not sufficient to have good doctors if cl<strong>in</strong>ics and hospitals<br />

are not properly managed or if nurses and other medical personnel are
