the trade <strong>in</strong> health services. It is therefore important for all countries <strong>in</strong>volved <strong>in</strong><br />

the <strong>in</strong>tern<strong>at</strong>ional trade <strong>in</strong> health services to particip<strong>at</strong>e actively <strong>in</strong> <strong>in</strong>tern<strong>at</strong>ional<br />

rule mak<strong>in</strong>g, implement<strong>at</strong>ion, and enforcement.<br />

Conclusion<br />

This chapter aims to help policy makers <strong>in</strong>crease their understand<strong>in</strong>g of the trade<br />

<strong>in</strong> the health service phenomenon by provid<strong>in</strong>g guidel<strong>in</strong>es on ways to foster the<br />

trade <strong>in</strong> health services and, most important, the benefits of this trade. Some of<br />

the key steps th<strong>at</strong> are described <strong>in</strong> this chapter and th<strong>at</strong> should be taken before<br />

mak<strong>in</strong>g decisions perta<strong>in</strong><strong>in</strong>g to a country’s particip<strong>at</strong>ion <strong>in</strong> the global trade <strong>in</strong><br />

health services are <strong>in</strong>dic<strong>at</strong>ed below. The chapter provides guidance to countries <strong>in</strong><br />

address<strong>in</strong>g key issues and mak<strong>in</strong>g optimal choices with regard to trade and public<br />

health.<br />

• Wh<strong>at</strong> is the balance between the potential benefits and the costs of <strong>in</strong>creased<br />

trade <strong>in</strong> health services For each of the four modes of service delivery identified<br />

<strong>in</strong> the GATS (crossborder, consumption abroad, foreign establishment,<br />

and the movement of persons), wh<strong>at</strong> are the expected benefits and costs of<br />

<strong>in</strong>creased imports and wh<strong>at</strong> are the expected benefits and costs of <strong>in</strong>creased<br />

exports<br />

• Wh<strong>at</strong> are the accompany<strong>in</strong>g measures th<strong>at</strong> will help maximize the benefits and<br />

m<strong>in</strong>imize the costs of <strong>in</strong>creased trade These measures <strong>in</strong>clude avoid<strong>in</strong>g an<br />

<strong>in</strong>ternal and external human resource dra<strong>in</strong>, preserv<strong>in</strong>g universal access and<br />

other legitim<strong>at</strong>e public health objectives, avoid<strong>in</strong>g a two-tier health system,<br />

improv<strong>in</strong>g quality and widen<strong>in</strong>g the access to health services, promot<strong>in</strong>g transfers<br />

of technology and expertise, and <strong>in</strong>creas<strong>in</strong>g the human and f<strong>in</strong>ancial<br />

resources <strong>in</strong> health care. The measures <strong>in</strong>volved should cover each of the delivery<br />

modes, as well as imports and exports.<br />

There is a need to identify the strengths, weaknesses, opportunities, and risks of<br />

the country <strong>in</strong> the health service sector. When consider<strong>in</strong>g how to remedy weaknesses,<br />

it is necessary to fix the fundamentals (educ<strong>at</strong>ion, tra<strong>in</strong><strong>in</strong>g, <strong>in</strong>frastructure,<br />

and technology) and to adapt the domestic regul<strong>at</strong>ory framework (regul<strong>at</strong>ory<br />

audit and the reform of the rules th<strong>at</strong> are more trade restrictive than necessary to<br />

achieve legitim<strong>at</strong>e public health objectives). Ma<strong>in</strong>ta<strong>in</strong><strong>in</strong>g the strengths <strong>in</strong>volves<br />

design<strong>in</strong>g an export promotion str<strong>at</strong>egy (f<strong>in</strong>d<strong>in</strong>g the right niches, tapp<strong>in</strong>g the<br />

right markets) and improv<strong>in</strong>g coord<strong>in</strong><strong>at</strong>ion <strong>in</strong> the public sector and with the priv<strong>at</strong>e<br />

sector (improv<strong>in</strong>g st<strong>at</strong>istics and cre<strong>at</strong><strong>in</strong>g a horizontal structure for cooper<strong>at</strong>ion,
