1. Any individual (e.g., athletic department staff member, student-athlete, university
employee, athletic representative) may report an alleged, rumored, or suspected
violation.
2. The individual may report the alleged, rumored, or suspected violation verbally,
in writing, or anonymously.
B. To Whom Shall An Alleged Violation Be Reported
1. If an alleged or rumored violation is conveyed to any athletic department staff
member, the Assistant Athletic Director for Compliance is required to notify the
Director of Athletics of the possible violation immediately.
2. Individuals other than athletic department staff members may report an alleged
violation to any or all of the following persons:
a. Director of Athletics
b. Assistant Athletic Director for Compliance
c. Faculty Athletics Representative
C. Conducting An Investigation Of A Violation
1. The Director of Athletics is responsible for the conduct of all investigations. The
Director of Athletics, in connection with the Chancellor and Faculty Athletics
Representative, may request formation of a committee to conduct or assist an
investigation of an actual or alleged violation.
2. The Director of Athletics shall maintain a written record of all alleged violations.
The record shall consist of the following:
a. Date that the alleged violation was reported to the Director of Athletics or
Assistant Athletic Director for Compliance and by whom the violation was
reported, if not by an anonymous source.
b. Detailed summary of the nature of the alleged violation, including the names
of all persons involved in the alleged violation.
c. Chronology of actions taken by the Director of Athletics in the investigation.
d. Rationale for concluding whether the allegation was or was not a violation of
NCAA regulations.
e. If a violation has occurred, the Director of Athletics will keep on file a copy of
the report of the violation and subsequent correspondence with the NCAA
