AVALIAÇAO BIOQUÍMICA E DE ESTRESSE OXIDATIVO EM PACIENTES COM DOENÇA DO XAROPE DO BORDO<br />
DURANTE O TRATAMENTO DIETÉTICO<br />

Introdução: Na Doença do Xarope do Bordo (DXB) a ativi<strong>da</strong><strong>de</strong> do complexo <strong>de</strong>sidrogenase dos a-cetoácidos <strong>de</strong> ca<strong>de</strong>ia ramifica<strong>da</strong><br />
é <strong>de</strong>ficiente, resultando no acúmulo <strong>de</strong> leucina, isoleucina e valina, e seus correspon<strong>de</strong>ntes a e hidroxi -cetoácidos. A terapia para<br />
esta doença consiste em uma dieta restrita em proteínas e suplementa<strong>da</strong> com uma fórmula <strong>de</strong> aminoácidos essenciais exceto<br />
leucina, isoleucina e valina. Estudos em humanos mostraram que o estresse oxi<strong>da</strong>tivo ocorre em pacientes com DXB no momento<br />
do diagnóstico.Objetivos: realizar uma avaliação e correlação do perfil bioquímico e <strong>de</strong> estresse oxi<strong>da</strong>tivo dos pacientes com<br />
Doença do Xarope do Bordo durante o tratamento. Material e Métodos: Foram avaliados a glicemia, o perfil lipídico, a função<br />
renal e a ativi<strong>da</strong><strong>de</strong> <strong>da</strong>s transaminases, bem como a medi<strong>da</strong> <strong>da</strong>s espécies reativas ao ácido tiobarbitúrico (TBA-RS) e a reativi<strong>da</strong><strong>de</strong><br />
antioxi<strong>da</strong>nte total (TAR) em soro <strong>de</strong> pacientes DXB tratados e <strong>de</strong> indivíduos saudáveis (controles). Resultados: Foi observado que<br />
nos pacientes com DXB sob tratamento ocorre diminuição dos níveis plasmáticos <strong>de</strong> glicose, albumina, creatinina e colesterol<br />
total e aumento <strong>da</strong> ativi<strong>da</strong><strong>de</strong> <strong>da</strong> aspartato amino transferase, bem como aumento do TBARS e uma diminuição do TAR. Foi<br />
observa<strong>da</strong> uma correlação significativa positiva entre os níveis <strong>de</strong> triglicerí<strong>de</strong>os e a medi<strong>da</strong> <strong>de</strong> TBARS. Conclusão: Nossos<br />
resultados sugerem que ocorre estresse oxi<strong>da</strong>tivo nos pacientes com DXB tratados e, ain<strong>da</strong>, que o tratamento dietético e as<br />
condições clínicas associa<strong>da</strong>s à doença po<strong>de</strong>m levar a alterações bioquímicas importantes. Apoio: CNPq, FAPERGS,<br />

MOLECULAR ANALYSIS OF COLORECTAL AND ENDOMETRIAL TUMORS FROM PATIENTS AT-RISK FOR LYNCH<br />
SYNDROME<br />

Intoduction: About 20% of colorectal tumors and 5% of endometrial tumors are hereditary. The recognition of at risk individuals<br />
for syndromes of hereditary predisposition to these tumors allows to implement strategies of specific prevention and<br />
accompaniment for these patients. Lynch Syndrome (also called HNPCC) is the most frequent form of hereditary colorectal<br />
cancer. In typical HNPCC families multiple generations are affected for CCR at early onset (~45 years). Endometrial cancer is the<br />
second malignance in afected families and is also the most frequent tumor in women who have the Syndrome. Microsatellite<br />
Instabilitry (MSI), Imunohistochemestry (IHQ) and Methylation Analysis are usefull to complete the clinical diagnosis of these<br />
patients. Objectives: To characterize colon and endometrial tumors from at risk patients for Lynch Syndrome throught MSI, IHC<br />
and Methylation analysis of MLH1, MSH2 and MSH6 genes and to correlate its presence with clinical markers. Materials and<br />
Methods: Patients with Bethes<strong>da</strong> criteria and patients with endometrial cancer diagnosed before the age of 50 years, are inclu<strong>de</strong>d.<br />
DNA was obtained from paraffin-embed<strong>de</strong>d normal and tumor tissue using MagneSil Genomic Fixed Tissue System kit<br />
(PROMEGA). To MSI analysis we use MSI Analysis System Version 1,1 kit (PROMEGA), that contains the pannel of the five<br />
recommen<strong>de</strong>d markers. IHC analysis is realized with specific antibodies against protein products of MLH1, MSH2 and MSH6<br />
genes. Methylation analysis is carried out using MS-MLPA technique with ME-MLH1 kit (MRC-Holland, Amster<strong>da</strong>m). Results<br />
and Conclusions: At the moment, 51 patients were inclu<strong>de</strong>d. From these, 11 of them have MSI analysis done, 40 have IHC and 5<br />
have Methylation Analysis complete. To the <strong>da</strong>te of the event, we expect to analyse the rest of the patients and conlcu<strong>de</strong> that this<br />
techniques are importants tools to complete the clinical diagnostic of these patients.<br />
EVOLUÇÃO TEMPORAL DA FUNÇÃO RENAL ENTRE PACIENTES CRITICAMENTE DOENTES: PAPEL DOS<br />
POLIMORFISMOS I/D E -262A>T DO GENE DA ENZIMA CONVERSORA DE ANGIOTENSINA (ECA). DISSERTAÇÃO<br />
DE MESTRADO EM BIOLOGIA CELULAR E MOLECULAR-PUCRS<br />

Introdução: A disfunção <strong>de</strong> múltiplos órgãos e a Insuficiência renal agu<strong>da</strong> compartilham muitos fatores fisiopatológicos. O gene<br />
<strong>da</strong> ECA po<strong>de</strong>ria ser um potencial candi<strong>da</strong>to a fator <strong>de</strong> risco genetico em pacientes <strong>de</strong> UTI. Objetivo: Determinar se há efeito dos<br />
polimorfismos I/D e -262A>T do gene <strong>da</strong> ECA na tendência <strong>da</strong> função renal em pacientes criticamente doentes. Material e<br />
Métodos: Um escore <strong>de</strong> avaliação seqüencial <strong>da</strong> disfunção orgânica (SOFA) foi empregado para <strong>de</strong>terminar o estado basal <strong>da</strong><br />
saú<strong>de</strong> no 1°dia <strong>de</strong> admissão à UTI do HSL-PUCRS <strong>de</strong> 153 pacientes adultos (79 homens). A tendência <strong>da</strong> função renal foi obti<strong>da</strong><br />
pelo SOFA renal (<strong>de</strong>terminado pela medi<strong>da</strong> diária <strong>da</strong> creatinina sérica e <strong>da</strong> diurese). Observou-se a progressão à disfunção renal<br />
(valores 3 e 4 <strong>de</strong> escores SOFA) nos primeiros 7 dias <strong>de</strong> admissão em UTI e necessi<strong>da</strong><strong>de</strong> <strong>de</strong> diálise durante este período. Outros<br />
<strong>de</strong>sfechos <strong>de</strong> interesse: óbito e tempo permanência em UTI e hospitalar. Foi obtido Consentimento Informado e aprovação em<br />
Comitê <strong>de</strong> Etica. Resultados: As freqüências genotípicas totais em nossa amostra foram II=0.17; ID=0.46; DD=0.37 e AA=0.30;<br />
AT=0.55; TT=0.15, e as freqüências alélicas foram I=0.40; D=0.60 e A=0.56; T=0.44. Nenhuma associação significativa foi<br />
encontra<strong>da</strong> entre a evolução <strong>da</strong> função renal no período estu<strong>da</strong>do e os genótipos ou freqüências alélicas dos polimorfismos em<br />
questão. Conclusão: Os polimorfismos I/D e -262A>T não apresentaram nenhum impacto significativo na tendência <strong>da</strong> função<br />
renal durante a primeira semana <strong>de</strong> internação na UTI. Não houve influência <strong>da</strong> herança genética dos mesmos sobre a mortali<strong>da</strong><strong>de</strong><br />
nos pacientes estu<strong>da</strong>dos. Este é o primeiro estudo <strong>de</strong>lineado para verificar a influência <strong>de</strong> polimorfismos I/D e -262A>T do gene<br />
ECA na disfunção renal agu<strong>da</strong> entre pacientes <strong>de</strong> UTI.
