Kostråd for å fremme folkehelsen og forebygge kroniske sykdommer vandrergrupper, særlig pakistanere, er en høyrisikogruppe for vitamin D-mangel. Siden solens betydning for vitamin D-produksjon i huden er mindre hos mange innvandrergrupper, er kosten og tilskudd ekstra viktig for deres vitamin D-status. Mange eldre kan også ha dårlig vitamin D-status, og særlig utsatt er hjemmeboende med lavt funksjonsnivå og de som bor på institusjon. Vitamin D-status blir dårligere med alderen på grunn av ineffektiv syntese i huden, samt redusert matinntak og utendørs aktivitet (4,5). Blant dem som spiser lite fet fisk, lite matvarer tilsatt vitamin D og som ikke tar vitamin D-tilskudd, er inntaket av vitamin D så lavt at det kan føre til utilfredsstillende vitamin D-status dersom de ikke produserer nok vitamin D ved solbelysning av huden. Siden dårlig vitamin D-status er vanlig blant ikke-vestlige innvandrere, anbefaler Helsedirektoratet (65,66) at de bør ta vitamin D-holdige tilskudd (7,5-10 mikrogram per dag) hele året. Dette er særlig viktig for gravide, spedbarn og barn. Helsedirektoratet anbefaler også at eldre personer som er lite ute i dagslys, bør få tilskudd med 10 mikrogram vitamin D per dag i tillegg til inntaket fra kost-holdet. Dette kan være som et rent vitamin D-tilskudd, som 5 ml tran eller tilsvarende i trankapsler. Helsedirektoratet (65) anbefaler for den generelle befolkningen: ”For de fleste vil solbelysing av huden dekke behovet for vitamin D i sommerhalvåret. Når produksjonen i huden ikke er tilstrekkelig, kan man dekke behovet via kosten. Alternativet for de som er lite ute i sollys, og som har lavt inntak av fet fisk og matvarer tilsatt vitamin D, er å ta tilskudd med vitamin D”. Svenske helsemyndigheter (79) legger til grunn at kostholdsstudier i Sverige finner at de aller fleste får i seg tilstrekkelige mengder vitaminer og mineraler, og at de fleste derfor ikke har behov for ekstra kosttilskudd. Imidlertid vil noen grupper ha behov for kosttilskudd i visse perioder av livet. Det gjelder vitamin D til barn opp til toårsalder og eldre over 70 år, spesielt til individer som ikke regelmessig oppholder seg utendørs. Tilskudd med multivitaminer/multimineraler Konsensusrapporten ”Multivitamin/Mineral Supplements and Chronic Disease Prevention” fra ”National Institutes of Health” (se faktaboks 15.5) fra 2007 (19) konkluderer at det ikke er tilstrekkelige dokumentasjon for verken å anbefale eller å fraråde bruk av multivitamin/mineraltilskudd for å forebygge kroniske sykdommer. Tilskudd med lange flerumettede omega-3-fettsyrer (EPA, DHA) På oppdrag fra EU-kommisjonen har ”European Food Safety Authority” utarbeidet nye næringsstoffanbefalinger for fett (18). EFSA anbefaler at et adekvat inntak av lange flerumettede omega-3-fettsyrer (EPA, DHA) bør være 250 mg/dag i friske individer. Gravide kvinner og kvinner som ammer, anbefales et tillegg på 100-200 milligram DHA per dag. FAO/WHO 2009-rapporten (72) anbefaler et inntak mellom 0,250 og 2 g langkjedete flerumettede omega-3-fettsyrer (DHA+EPA) per dag (se faktaboks 15.4), 5. Litteratursøk I hovedsak er det benyttet de systematiske kunnskapsoppsummeringene som det er redegjort for i kapittel 4.
