L’ultima considerazione sull’utilizzo di cemento per lo stelo
riguarda un caso particolare: la protesi inversa abbinata
al transfer muscolare di gran rotondo e gran dorsale, soluzione
che dal 2003 adottiamo nei casi di defi cit completo
della cuffi a posteriore, al fi ne di permettere un parziale
recupero dell’extrarotazione.

In questa situazione il transfer muscolare viene fi ssato a
livello metafi sario, con l’utilizzo di suture trans ossee fatte
passare attraverso la metafi si prima di impiantare lo stelo.
Con questa metodica l’utilizzo di uno stelo cementato è
preferibile in quanto un sistema a press fi t potrebbe tranciare
le suture che attraversano il canale (Figg. 9, 10).
