zione ra<strong>di</strong>ografi ca postoperatoria ha evidenziato che la<br />
sezione conica prossimale con un maggiore cono anteroposteriore<br />
(14°) dello stelo ha consentito <strong>di</strong> ottenere un<br />
ottimale riempimento del canale femorale metafi sario sia<br />
me<strong>di</strong>o-laterale (94%) che antero-posteriore (91%). Questa<br />
morfologia ed il rivestimento prossimale in trabecular<br />
metal ha permesso <strong>di</strong> realizzare una valida fi ssazione<br />
primaria anche in femori cilindrici (tipo C <strong>di</strong> Dorr) come<br />
<strong>di</strong>mostrato dall’assenza <strong>di</strong> casi <strong>di</strong> affondamento dello stelo<br />
(Fig. 1). L’analisi ra<strong>di</strong>ografi ca della fi ssazione secondaria<br />
dello stelo ha evidenziato che erano presenti ponti<br />
ossei nelle zone 2-6-9-13 a <strong>di</strong>stanza <strong>di</strong> 12 mesi in 128<br />
anche su 180 (71%) e nel 100% <strong>di</strong> casi valutati a 24<br />
mesi. In 17 anche (9,4%) sono state riscontrati segni <strong>di</strong><br />
rimodellamento del femore <strong>di</strong>stale con comparsa <strong>di</strong> linee<br />
reattive dell’osso spongioso intorno alla punta dello stelo,<br />
associati ad ipertrofi a dell’osso corticale (2 anche) e/o<br />
formazione <strong>di</strong> un pie<strong>di</strong>stallo (2 anche). In questi pazienti<br />
i segni ra<strong>di</strong>ografi ci non erano associati a dolore <strong>di</strong> coscia,<br />
l’analisi DEXA non ha evidenziato un aumento della<br />
densità minerale ossea periprotesica e pertanto possono<br />
essere interpretati come reazione secondaria dell’osso a<br />
micromovimenti <strong>di</strong>stali dello stelo che, avendo una superfi<br />

lo TMP ha evidenziato che complessivamente a 24 mesi il<br />
patrimonio osseo del femore viene preservato con un riduzione<br />
me<strong>di</strong>a della DMO totale del 5,1% rispetto al valore<br />
basale postoperatorio. Il rivestimento in trabecular metal<br />
realizza una rapida osteointegrazione prossimale dell’impianto<br />
con un progressivo recupero della densità ossea<br />
in zona 1,2 e 6 già dopo 12 mesi e che prosegue fi no<br />
ai 24 mesi con il raggiungimento dei valori basali (zona<br />
1: -1,3%, zona 2: -1,4%, zona 6: -0,2%) anche in femori<br />
cilindrici con ridotta qualità ossea preoperatoria. In conclusione<br />
possiamo affermare che il rivestimento in trabecular<br />
metal dello stelo TMP contribuisce a realizzare una<br />
valida stabilità primaria ed una fi ssazione prevalentemente<br />
metafi saria. L’osteointegrazione metafi saria della componente<br />
femorale riduce lo stress shiel<strong>di</strong>ng e rappresenta<br />
una barriera alla migrazione <strong>di</strong> possibili detriti articolari<br />
che possono determinare una mobilizzazione asettica a<br />
<strong>di</strong>stanza. Risulta comunque necessario una valutazione<br />
clinica e ra<strong>di</strong>ografi ca a lungo termine per confermare se<br />
il rivestimento in trabecular metal dello stelo rappresenti<br />
effettivamente un vantaggio nella sua fi ssazione e possa<br />
quin<strong>di</strong> rappresentare una meto<strong>di</strong>ca utile per migliorare i<br />
risultati clinici e la sopravvivenza degli impianti non cementati<br />
anche nei pazienti con ridotta densità minerale<br />
ossea.<br />
