riassunto<br />
Background: La cartilagine ialina è un tessuto privo <strong>di</strong> terminazioni<br />
nervose e vasi linfatici, avascolare e con una scarsa<br />
capacità rigenerativa. Attualmente, sono <strong>di</strong>sponibili <strong>di</strong>verse tecniche<br />
per il trattamento delle lesioni della cartilagine articolare:<br />
tecniche <strong>di</strong> stimolazione midollare, come le microfratture, e tecniche<br />
<strong>di</strong> ricostruzione con tessuto autologo od omologo, quali<br />
la mosaicoplastica e gli allotrapianti osteocondrali. Tra queste<br />
l’impianto <strong>di</strong> condrociti autologhi (ACI) è <strong>di</strong>ventato una procedura<br />
<strong>di</strong> frequente utilizzo.<br />
obiettivo: Presentare le attualità sugli ACI dalle in<strong>di</strong>cazioni alla<br />
riabilitazione anche in relazione alle altre tecniche per il trattamento<br />
delle lesioni cartilaginee <strong>di</strong>sponibili al momento.<br />
risultati: In letteratura vengono presentati buoni risultati con i<br />
<strong>di</strong>versi trattamenti per le lesioni condrali, senza però evidenziare<br />
una significativa superiorità dell’ACI rispetto alle altre<br />
procedure.<br />
conclusioni: Attualmente non è possibile concludere per un’in<strong>di</strong>cazione<br />
assoluta all’utilizzo degli ACI nel trattamento delle lesioni<br />
condrali, anche in relazione alla maggiore invasività rispetto<br />
ad altre tecniche, data la necessità <strong>di</strong> due interventi chirurgici e<br />
i lunghi tempi <strong>di</strong> recupero.<br />

Background: The hyaline cartilage of the articular surface is an<br />
aneural, avascular, and alymphatic tissue with a limited intrinsic<br />
healing potential. The current surgical treatment options utilized<br />
by orthopae<strong>di</strong>c surgeon in the management of chondral defects<br />
include lavage and shaving chondroplasty, marrow stimulation<br />
procedures (microfracture), osteochondral grafting techniques<br />
(mosaicplasty, osteochondral allograft implantation, osteochondral<br />
autograft transfer system [OATS]), and cell-based repair<br />
methods (autologous chondrocyte implantation). Among these,<br />

autologous chondrocytes implantation (ACI) has become a frequently<br />
used procedure.<br />
objective: To present the up-to-date on ACI from in<strong>di</strong>cations to<br />
rehabilitation also in relation with other treatments of chondral<br />
defects available at the moment.<br />
results: In the literature we have found good results with the <strong>di</strong>fferent<br />
treatments of chondral defects, without showing a significative<br />
superiority for the ACI over the other procedures.<br />
conclusions: Nowadays it is not possible to conclude for an absolute<br />
in<strong>di</strong>cation of the ACI in the treatment of chondral lesions,<br />
also because of the greater invasivity of the two-stage procedure<br />
and the long recovery time.<br />

introduZionE<br />
Le lesioni cartilaginee sono molto comuni nella popolazione<br />
e la loro incidenza aumenta progressivamente con<br />
l’età dei pazienti, variando nelle <strong>di</strong>verse casistiche tra<br />
l’11 e il 63% per quanto riguarda il ginocchio 1 2 . Sfortunatamente<br />
la cartilagine articolare è un tessuto dotato <strong>di</strong><br />
scarsa capacità rigenerativa, per la ridotta vascolarizzazione<br />
e la limitata <strong>di</strong>sponibilità <strong>di</strong> cellule in<strong>di</strong>fferenziate.<br />
Ad una lesione condrale traumatica o degenerativa può<br />
seguire un tentativo spontaneo <strong>di</strong> riparazione con tessuto<br />
fibrocartilagineo o l’evoluzione più o meno graduale verso<br />
l’artrosi.<br />
Per il trattamento <strong>di</strong> queste lesioni sono state proposte<br />
varie procedure sia <strong>di</strong> tipo riparativo che ricostruttivo 1 3 .<br />
Tra le prime troviamo le microfratture dell’osso subcondrale,<br />
proposte da Steadman con lo scopo <strong>di</strong> favorire<br />
il sanguinamento e il conseguente arrivo <strong>di</strong> precursori<br />
della cartilagine dal midollo osseo al sito <strong>di</strong> lesione 4 5 .<br />
Le procedure ricostruttive possono invece essere <strong>di</strong> tipo<br />
autologo come la mosaicoplastica e il trapianto <strong>di</strong> condrociti<br />
autologhi od omologhe con l’utilizzo <strong>di</strong> trapianti<br />
osteocondrali 6 7 .<br />
L’impianto <strong>di</strong> condrociti autologhi è stato il primo approccio<br />
biologico per il trattamento delle lesioni condrali del<br />
ginocchio ed è stato introdotto nel 1994 da Brittberg e<br />
Peterson 8 .<br />
tEcnica chirurgica<br />
Si tratta <strong>di</strong> una tecnica che prevede due tempi chirurgici.<br />
Il primo intervento, solitamente artroscopico, serve per<br />
la valutazione dell’in<strong>di</strong>cazione all’utilizzo <strong>di</strong> ACI e per<br />
il conseguente prelievo <strong>di</strong> tessuto cartilagineo sano in<br />
un’area <strong>di</strong> ridotto carico.<br />
La sede più comune per il prelievo cartilagineo è il margine<br />
supero me<strong>di</strong>ale del con<strong>di</strong>lo femorale me<strong>di</strong>ale e il<br />
margine laterale della gola intercon<strong>di</strong>loidea 9 . La quantità<br />
<strong>di</strong> tessuto da prelevare varia dai 200 mg ai 300 mg.<br />
I condrociti vengono quin<strong>di</strong> isolati me<strong>di</strong>ante <strong>di</strong>gestione<br />
enzimatica ed espansi in coltura per un periodo variabile<br />
tra le 2 e le 6 settimane 8 10 .
