dönemde kreatinin yüksekliği, laktik asidoz<br />
ortaya çıkabilmektedir. Oral antiviralleri en sık<br />
bırakma nedenleri baş ağrısı ve üst solunum<br />
yolu yakınmalarıdır. PEG-IFN başta nötropeni<br />
olmak üzere pek çok istenmeyen etkiye sahiptir.<br />
Psikolojik sorunlar geliştirmesi de azımsanmayacak<br />
kadar çoktur. Tüm tedavilerin karşılaştırıldığı<br />
bir meta-analizde; depresyon en sık (%5)<br />
PEG-IFN tedavisi alan HBV hastalarında gözlenmiştir.<br />
Aynı çalışmada istenmeyen etkiler<br />
nedeniyle tedaviyi bırakma sıklıkları sırasıyla<br />
PEG-IFN (%5), lamivudin (%2.8), entekavir<br />
(%1.8), tenofovir (%1) ve adefovir (%1) olarak<br />
bildirilmiştir. Karaciğer enzimlerinde dalgalanmalar<br />
ise sırasıyla entekavir (%1), telbivudin<br />
(%1), adefovir (%2), lamivudin (%4), tenofovir<br />
(%6), lamivudun + adefovir (%7) ve PEG-IFN<br />
(%8) olarak saptanmıştır.<br />
PEG-IFN’nin özellikle kadın, genç, yüksek ALT<br />
ve düşük HBV-DNA değerine sahip ve komorbid<br />
hastalığı olmayanlarda daha başarılı olduğu bildirilmektedir.<br />
Ancak bu hasta gruplarında<br />
nükleoz(t)id analoglarına göre özel bir üstünlüğü<br />
bulunmamaktadır. PEG-IFN, nükleoz(t)id<br />
analoglarından farklı olarak, dekompanse sirozda<br />
kontrendikedir ve kompanse siroz, gebe ve<br />
immünyetmezlik durumlarında kullanılmaları<br />
önerilmemektedir.<br />
Sonuç olarak; olası tedavi seçeneklerini etkinlik,<br />
güvenlik, direnç geliştirme riski ve tedavi maliyeti<br />
açısından değerlendirmek ve tedaviye başlarken<br />
mutlaka hastanın özelliklerini de hesaba<br />
katmak en akılcı yöntemdir.<br />
