6. Substitutions or "equivalent" Items
A. The Contract, if awarded, will be on the basis of materials and equipment described in the
Drawings or specified in the Specifications without consideration of possible substitute or "orequivalent"
items. Whenever it is indicated in the Drawings or specified in the Specification
that a substitute or "or-equivalent" item of material or equipment may be furnished or used by
Bidder if acceptable to Engineers/CMAR, application for such acceptance will not be
considered by Engineers/CMAR until after the Effective Date of the Agreement unless the
Engineers/CMAR at their option, elect to issue an Addenda naming a substitute or "orequivalent"
item prior to receipt of bids. For EQUIPMENT PROPOSALS only the
Manufacturers listed in the Specifications will be accepted. The procedure for submission of
any such application by Bidder and consideration by Engineers/CMAR as set forth in
Paragraph 6.05 of the General Conditions and in CMAR’s Specification Section 01 33 00,
Submittal Procedures.
7. Bid Form
A. All blanks on the Bid Form must be completed in ink or by typewriter. Bidder shall indicate
with “N/A” any Bid Packages not being proposed on.
B. Bids by corporations must be executed in the corporate name by the president or a vicepresident
(or other corporate officer accompanied by evidence of authority to sign) and the
corporate seal must be affixed and attested by the secretary or an assistant secretary. The
corporate address and state of incorporation must be shown below the signature.
C. Bids by partnerships must be executed in the partnership name signed by a partner, whose
title must appear under the signature and the official address of the partnership must be
shown below the signature.
D. All names must be typed or printed below the signature.
E. The Bid shall contain an acknowledgment of receipt of all Addenda (the numbers of which
must be filled in on the Bid Form).
F. The address and telephone number for communications regarding the Bid must be shown.
G. Bids must be priced as indicated on the Bid Form for the Bid Packages as applicable to the
Bidder. If applicable the price of the alternative bids for each Bid Package will be the amount
to be added to or deducted from the price of the Package’s Base Bid if the alternate is
selected.
8. Submission of Bids
A. Submission of Bid shall include:
1. Bid Proposal Package
2. Attachment A – Bid Proposal
3. Attachment B – Bidder’s Profile
4. Attachment C – Bidder’s Statement of Experience.
5. Attachment D – Subcontractor’s Safety Performance Questionnaire.
6. Attachment E - Bidder’s Scope of Work/Supply.
7. Attachment F – List of Subcontractors
8. Attachment G – List of Suppliers
