Chapter 2 – Mission Planning<br />

G.1. Unit<br />

Responsibilities<br />

The unit CO/OIC shall:<br />

(01) Appoint a Safety Officer (typically XO/XPO in accordance with<br />

Reference (i).<br />

(02) Appoint a Unit Safety Coordinator (USC). The USC shall be<br />

formally trained and capable of coordinating safety activities<br />

across the scope of the command.<br />

(03) Ensure safety prevention and risk management measures are<br />

conducted, including:<br />

a) Communicate in writing to all hands the periodic meeting of<br />

the cognizant Unit Safety Board, hazards identified and<br />

mitigating strategies.<br />

b) Communicate in writing to the chain of command safety<br />

issues / mitigation strategies that are beyond the unit’s<br />

capability to control (e.g. funding, training, etc.).<br />

c) Document the identification and mitigation of safety<br />

hazards via periodic self safety inspections.<br />

d) Obtain formal safety assessments via HSWL Service<br />

Center, Safety and Environmental Health Division.<br />

e) Maintain the various safety programs required by Reference<br />

(h) (e.g. OMSEP, Respiratory Protection, Hearing<br />

protection, etc.).<br />

f) Conduct safety related training.<br />

(04) Annually conduct unit-wide exercise of the Unit Pre-Mishap<br />

Plan. Respond to MISHAPs using Pre-Mishap Plan.<br />

(05) Investigate, evaluate, and report all mishaps.<br />

a) Parent units shall conduct mishap investigations of<br />

subordinate units.<br />

b) Clearly identify specific contributing and causal factors.<br />

(Human Factors Analysis Codes (HFACS) are an<br />

excellent tool for this purpose).<br />

c) Mishaps shall be entered into EMISHAP system.<br />

(06) Review unit’s self and formal safety assessments as part of<br />

change of command process.<br />
