Section I. MAB Procedure and Mishap Policy<br />

Introduction<br />

I.1. MAAIT<br />

This section discusses Mishap Assessment Board (MAB) procedures,<br />

Mishap policy, and the Boat Forces Mishap Analysis, Assistance and<br />

Investigation Team (MAAIT).<br />

The Office of Boat Forces Commandant (CG-731) shall train and equip<br />

selected Boat Forces personnel to serve as a MAAIT to respond<br />

immediately to mishaps for the purpose of gathering and protecting<br />

information and evidence in advance of Commandant-directed MAB<br />

investigations. Additionally, Commandant (CG-731) and the Office of<br />

Afloat Safety Commandant (CG-1134) will detail the use of the<br />

MAAIT’s expertise when needed by Boat Forces command cadre to aid<br />

them in conducting a quality mishap investigation for all classes of<br />

mishaps, High Potential (HIPO) mishaps, and near misses. Commandant<br />

(CG-731) shall provide team assistance to requesting commands.<br />

The Boat Forces MAAIT shall provide the mishap investigation<br />

personnel for all boat-related MAB investigations, as appropriate.<br />

Additionally, members shall serve as the senior Subject Matter Expert<br />

(SME) advisory board to <strong>Coast</strong> <strong>Guard</strong> leadership regarding all classes of<br />

Boat Force mishaps.<br />

I.1.a. Membership The MAAITis co-chaired by Commandant (CG-731) and Commandant<br />

(CG-1134). The team membership must reflect the highest level of Boat<br />

Forces knowledge, professionalism, and personal conduct. To achieve<br />

adequate coverage, the council will work to ensure there are at least three<br />

(3) members per district, and one (1) member each from Boat Forces<br />

Center (TCY), Boat Forces Doctrine (TCY), National Motor Lifeboat<br />

School (NMLBS), and Special Missions Training Center (SMTC). The<br />

desired prerequisites for membership are:<br />

(01) Paygrades of E-7 to W-4 and O-4 to O-6.<br />

(02) Current or previous Boat Forces experience.<br />

The council should convene at least annually to conduct a review and<br />

discuss Boat Forces mishaps.<br />
