All reports shall be made on 8-1/2 x 11 white papers, suitable for photocopying and binding in
booklet form. All sheets shall have the Testing and Engineer’s letterhead (including phone number,
address and email address). Larger sheets shall be folded and bound into the booklet. Reports
shall include:
a. Date issued
b. Project title and number as it appears on the Contract Documents.
c. Name of the inspector
d. Name and seal of registered engineer in responsible charge.
e. Date and time of sampling or inspection.
f. Identification of product and specification section.
g. Location of sampling or inspection in the Project.
h. Type of inspection or test performed.
i. Date of test.
j. Results of test.
k. Indicate conformance or non-conformance with the Contract Document requirements and
provide Testing and Engineer’s interpretation of test or inspection results.
All tests and inspections not conforming to the Contract Documents shall be stamped with a red
stamp indicating non-0conrormance and distributed within 20 hours of discovery using the red letter
format attached to this document.
After each inspection or test, a report shall be distributed within three calendar days to:

3.10 Structural Inspection Services Shall Include:
Concrete Foundations and Slabs-on-Grade:
a. Inspect foundations for compliance with Drawings and Specifications. Report on the following:
• Concrete footing size and depth.
• Footing rebar size, spacing and placement (cover).
• Placement and vibration of concrete.
• Dowel bar size, orientation, embedment and spacing.
• Anchor bolt size, orientation, embedment and spacing.
b. Inspect slabs-on-grade for compliance with Drawings and Specifications. Report on the
following:
• Preparation of subgrade.
• Slab thickness.
• Size, spacing, placement (cover) and lap of reinforcement, if applicable.
• Size, spacing and placement of joint dowels.
• Placement and finishing of concrete.
• Time of saw cuts after placement of concrete.
Masonry Walls:
a. Inspect walls for compliance with Drawings and Specifications. Report on the following:
• Placement of concrete masonry units (joint space, level, plumb).
• Horizontal reinforcing, spacing and lap.
• Vertical bar size, spacing and placement (spacing across width of wall) at walls and control
joints.
• Vertical bar laps.
