Appendix IAffidavit of Unauthorized Transactions1. An individual <strong>Navy</strong> / Marine <strong>Cash</strong> cardholder has every right to dispute an unauthorizedATM withdrawal ashore or MasterCard debit purchase ashore if he or she suspects unauthorizedactivity on his or her <strong>Navy</strong> / Marine <strong>Cash</strong> card.2. In the event of an unauthorized transaction with a <strong>Navy</strong> / Marine <strong>Cash</strong> card, theindividual cardholder should stop using his or her card and immediately notify the CustomerService Center (CSC)Phone: 1 866 3NAVY CASH1 866 362-8922Web site: www.<strong>Navy</strong><strong>Cash</strong>.come-mail: <strong>Navy</strong><strong>Cash</strong>@ezpaymt.comFax: 1 866 CHASE011 866 242-73013. In order to dispute an unauthorized transaction, an individual must fill out, sign, and datean affidavit form. The form must then be faxed, mailed, or scanned and e-mailed to the CSC. If at all possible, the form should be sent to the CSC on the same day that theunauthorized use of the <strong>Navy</strong> <strong>Cash</strong> card was reported. The CSC will respond within 10 businessdays of receiving the affidavit form.5. The individual must fill out all applicable information on the form for the claim to beprocessed. If the individual has exercised reasonable care in safeguarding the <strong>Navy</strong> / Marine<strong>Cash</strong> card, he or she will not be responsible for unauthorized purchases, depending, of course, onthe history of the account, e.g., the individual has not reported two or more unauthorized eventsin the past 12 months. After the claim has been investigated and processed, the CSC will returnthe funds to the individual’s <strong>Navy</strong> / Marine <strong>Cash</strong> account.6. In the event an ATM ashore does not dispense the amount of money requested, i.e., toolittle or too much cash, no affidavit is required. The individual cardholder should immediatelynotify the CSC.
