agency working and audit <strong>of</strong> service use. There were variations in<br />

relation to assessment, communication and documentation practices<br />

and there was no systematic comprehensive multidisciplinary<br />

assessment in place. Discharge co-ordinators play a vital link in<br />

facilitating continuity <strong>of</strong> care. This study has identified core<br />

elements <strong>of</strong> the role <strong>of</strong> the discharge co-ordinators. The findings<br />

have important implications and provide insights into the complex<br />

role <strong>of</strong> discharge co-ordinators for nurses, managers and policy<br />

makers.<br />
