Objectives<br />

1) Describe motivational interviewing as a strategy to promote<br />

life change<br />

2) Discuss how motivational interviewing can be used with<br />

clinical populations<br />

3) Apply concepts <strong>of</strong> motivational interviewing to nursing<br />

practice<br />

Background and context:<br />

As a chronic disease with serious co-morbidities, diabetes care<br />

accounts for over $130 billion U.S. health care dollars per year<br />

because on average, expenses for a person with diabetes are six<br />

times those <strong>of</strong> a healthy person. Hypertension and hyperlipidemia,<br />

two common diabetic co-morbidities, can lead to increased mortality<br />

and further escalate the cost <strong>of</strong> care. However, even a 1% drop in<br />

HbA1c can lead to a 33% reduction in microvascular complications.<br />

In addition to physical complications, diabetes adversely affects<br />

patient quality <strong>of</strong> life, with clinical depression occurring more<br />

frequently in those with diabetes. Since cognitive function<br />
