Impact of a discharge coordinator on the sucessful discharge<br />

of elderly patients from the emergency department (ED),région de Montréal<br />

Impact d’un coordonnateur des congés <strong>au</strong> département d’urgence (DU)<br />

sur le succès <strong>du</strong> congé des patients âgés<br />

translate into a re<strong>du</strong>ction of cost to the health care system.<br />

To our knowledge this is the first study to address discharge of the elderly patient from the ED using a comprehensive nursing model. This<br />

study demonstrated the benefits of having a nurse dedicated to the discharge of elderly patients from the emergency department.<br />

The nursing intervention model con<strong>du</strong>cted in this study has shown to improve quality of care by providing a 7 day safety net to the patient<br />

thereby contributing to the re<strong>du</strong>ction of unsche<strong>du</strong>led return visits to the ED within 14 days.<br />

The nursing model empowers nurses to utilize expert judgement and specialized knowledge in an independent and yet, collaborative<br />

advanced practice. The model sets the standard for the further development of a specialized expertise in emergency nursing which in turn<br />

will lead to the creation of integrated health care policies and protocols aimed at ensuring coordination and continuity of care for all high<br />

risk patients.<br />
