What’s temporarily unemployable?<br />

Temporary means the individual has a physical or<br />

mental incapacity that makes it impossible to work<br />

for a limited duration and is expected to be resolved<br />

with treatment. Temporary disabilities have no<br />

expiration date <strong>of</strong> less than 12 months.<br />

What verification is needed for unemployability<br />

status?<br />

Individuals are referred to the contracted medical<br />

provider using a LEADER generated ABP 1676,<br />

Employability Screening Appointment – Contracted<br />

Provider. The medical provider completes the<br />

employability screening and inputs the results to<br />

LEADER.<br />

• Individuals who are under treatment with a<br />

County/GRHCP/VA physician may request that<br />

the physician do the employability screening.<br />

Individuals are referred to a County/GRHCP/VA<br />

physician using an out <strong>of</strong> drawer ABP 1676P,<br />

Employability Screening Appointment –<br />

Contracted Provider. A form from the<br />

County/GRHCP/VA <strong>of</strong>fice may also be used as<br />

long as it provides the information required on the<br />

ABP 1676P.<br />

When the ABP 1676P is used for the<br />

County/GRHCP/VA physician the following apply:<br />

‣ The ABP 1676P must be returned by the due<br />

date.<br />

‣ If the individual cannot get an appointment<br />

with the County/GRHCP/VA physician by the<br />

due date, the individual must be referred to<br />

the medical provider.<br />

‣ The individual remains employable until the
