GR 44-221.16 Full<br />

Cancellation<br />

The ABP 4026, LEADER Voucher Cancellation<br />

Request, (an out <strong>of</strong> drawer form) is used to request<br />

cancellation <strong>of</strong> housing vouchers.<br />

When is a full cancellation <strong>of</strong> the ABP 295<br />

needed?<br />

A full cancellation is requested only when the original<br />

voucher was not used AND a full replacement is<br />

being issued. The original voucher must be attached<br />

to the ABP 4026.<br />

GR 44-221.17 Partial<br />

Cancellation<br />

When is a partial cancellation <strong>of</strong> the ABP 295<br />

needed?<br />

A partial cancellation is completed when a<br />

replacement voucher is being issued for the unused<br />

days on the voucher being canceled. The Cancel<br />

Date for the original voucher is the last night the<br />

individual stayed at the hotel.<br />

How are complaints regarding emergency<br />

housing benefits and procedures handled?<br />

Individuals who have problems/complaints regarding<br />

emergency housing benefits or any aspect <strong>of</strong> the<br />

handling <strong>of</strong> the emergency housing benefits process<br />

are referred to the District Homeless Liaison. The<br />

Liaison advises the individual <strong>of</strong> the GR Complaint<br />

procedures found in GR 40-103.5 if the complaint<br />

cannot be resolved by the Liaison.<br />

How are complaints regarding emergency<br />

housing facilities handled?<br />

The PA 479 is provided with each voucher issued.<br />

The ABP 479 provides a toll-free telephone number<br />

that may be used by the individual to report a<br />

complaint.<br />

Under what conditions can emergency aid to<br />

prevent eviction/foreclosure be issued?<br />

Emergency aid to prevent eviction/foreclosure may<br />

be issued for the period beginning 30 calendar days<br />

prior to the date <strong>of</strong> application and through the month<br />

<strong>of</strong> application, subject to the following conditions.
