1. Do you have a need that is prescribed by your
doctor, such as
• Special diet;
• Transportation need (such as four or more
doctor appointments each month);
• Distilled water; or
• Medical need for telephone.
2. Do you own your own home and have a need for
• Payment of property taxes;
• Payment of fire insurance premium; or
• Household repairs.
3. Do you have an offer for full time employment
and have a need for
Occupational or Trade Tools (may include items
such as required uniforms and safety shoes.)
4. Do you have a need for personal care items such
as (Intake only)
• Comb, razor, shampoo, soap toothbrush,
toothpaste, and washcloth. (Sanitary napkins
are not included in the kit but may be issued
upon request).
5. Do you have a need for meals out of the home?
• A physician must show that you have a
physical or metal condition that makes it
impossible for you to shop for and/or prepare
meals.
• You had a catastrophe and you are in need of
meals out because your cooking facilities have
been destroyed by fire, earthquake, or flood.
