1. Do you have a need that is prescribed by your<br />

doctor, such as<br />

• Special diet;<br />

• Transportation need (such as four or more<br />

doctor appointments each month);<br />

• Distilled water; or<br />

• Medical need for telephone.<br />

2. Do you own your own home and have a need for<br />

• Payment <strong>of</strong> property taxes;<br />

• Payment <strong>of</strong> fire insurance premium; or<br />

• Household repairs.<br />

3. Do you have an <strong>of</strong>fer for full time employment<br />

and have a need for<br />

Occupational or Trade Tools (may include items<br />

such as required uniforms and safety shoes.)<br />

4. Do you have a need for personal care items such<br />

as (Intake only)<br />

• Comb, razor, shampoo, soap toothbrush,<br />

toothpaste, and washcloth. (Sanitary napkins<br />

are not included in the kit but may be issued<br />

upon request).<br />

5. Do you have a need for meals out <strong>of</strong> the home?<br />

• A physician must show that you have a<br />

physical or metal condition that makes it<br />

impossible for you to shop for and/or prepare<br />

meals.<br />

• You had a catastrophe and you are in need <strong>of</strong><br />

meals out because your cooking facilities have<br />

been destroyed by fire, earthquake, or flood.<br />
