Making A Referral<br />

What needs to be done prior to making a Property<br />

<strong>Services</strong> referral?<br />

An investigation is initiated within five (5) working<br />

days <strong>of</strong> application or the receipt <strong>of</strong> information,<br />

whichever comes first.<br />

Before making a Property <strong>Services</strong> referral, as much<br />

information as possible must be obtained about all<br />

real and/or personal property currently owned within<br />

the last two years by the GR individual.<br />

Property <strong>Services</strong> referrals are usually initiated for<br />

property owned or disposed <strong>of</strong> within the last two<br />

years. However, property owned or disposed <strong>of</strong> prior<br />

to that time may also be referred.<br />

Referrals<br />

How are Hotline Property <strong>Services</strong> referrals<br />

made?<br />

• A call is made to the Hotline at (626) 854-4732 to<br />

request the investigation <strong>of</strong> urgent and noncomplex,<br />

real, and personal property.<br />

• The required verification information is provided.<br />

• The telephone discussion(s) and reported<br />

investigative findings are documented in the case<br />

comments section.<br />

• The written confirmation <strong>of</strong> the property<br />

investigation (sent by Property <strong>Services</strong> as a<br />

follow up to the Hotline referral) is filed in the<br />

Property folder.<br />

• The ABCDM 228 or PA 2465, Participant’s<br />

Consent for Release <strong>of</strong> Information from a<br />

Financial Institution, (an out <strong>of</strong> drawer form) is<br />

required for each item being requested.
