GR 45-115.1 Case Open<br />

in Another District<br />

What needs to be done when an individual<br />

currently receiving GR enters a B&C facility and<br />

the case is open in another district?<br />

When the case is in another district, the B&C Liaison<br />

responsible for handling the B&C facility (receiving<br />

district) notifies the B&C Liaison in the case-carrying<br />

district (sending district) by telephone or Board and<br />

Care Case Transfer Fax (an out <strong>of</strong> drawer) that the<br />

individual is now in a B&C in the receiving district’s<br />

service area.<br />

The sending district:<br />

• Immediately walks the case through the transfer<br />

desk to have the case transferred to the receiving<br />

district.<br />

• The case is not terminated.<br />

Refer to WA No. 40 Case Transfers for LEADER<br />

procedures on case transfers.<br />

• After the case has been transferred via LEADER,<br />

the case is walked to the district B&C Liaison who<br />

places the case in an envelope and sends it via<br />

County mail to the receiving B&C Liaison. A PA<br />

6-1, “Miscellaneous Transmittal” should<br />

accompany the case.<br />

NOTE: Every effort possible should be made to<br />

transfer the case within three workdays<br />

<strong>of</strong> the reported move.<br />

The receiving district:<br />

• B&C Liaison schedules a district <strong>of</strong>fice interview<br />

(or home call, if appropriate) within five workdays<br />

<strong>of</strong> receiving the information that the individual is<br />

in the facility.<br />

• At the time <strong>of</strong> the district <strong>of</strong>fice interview or home<br />

call, the Worker obtains the ABP 320.3 and any<br />

other information necessary to process the case.
