Related Injury or Illness<br />

When an individual reports a work related injury<br />

or illness, what should be done?<br />

When an individual is injured on the job or suffers<br />

an illness that is job-related, the individual is<br />

advised that a claim must be filed and the results<br />

reported to the Worker. (Claim forms and<br />

instructions for filing are provided by the employer.)<br />

What are the eligibility criteria for Workers’<br />

Compensation?<br />

Individuals who meet two <strong>of</strong> the following criteria<br />

may be eligible to Workers’ Compensation benefits<br />

and must be referred to the appropriate employer to<br />

apply within five working days. The individual must:<br />

• be <strong>of</strong>f work with doctor’s consent, and either<br />

• have been injured on the job, or<br />

• suffered an illness which is job-related.<br />

Verification<br />

Verification: The Worker views the documents<br />

listed below and records in Case Comments the<br />

date the verification is seen and all pertinent<br />

information such as the award/denial/appeal data.<br />

• Documents to be reviewed:<br />

‣ Evidence, such as a statement from the<br />

employer that the claim was filed, must<br />

be reviewed.
