Responsibilities Following are responsibilities <strong>of</strong> the district<br />

receptionist(s):<br />

• Ask all individuals if they need help with reading,<br />

writing or completing forms.<br />

• Do not give application forms to an individual<br />

who needs help completing them (if this is<br />

known beforehand) or to anyone who appears to<br />

be mentally disabled.<br />

• Annotate the upper right corner <strong>of</strong> intake<br />

materials “NSA” in red, when the individual<br />

needs help with reading and/or writing or<br />

appears to be mentally disabled.<br />

• Advise individuals who are determined to need<br />

special assistance that a Worker will provide<br />

help with the forms.<br />

• Inform individuals who are determined not to<br />

need special assistance that a Worker will help<br />

them complete any items on the forms that they<br />

do not understand or cannot complete.<br />

Worker Responsibilities<br />

How are individuals who are potentially mentally<br />

disabled assisted with the intake process?<br />

• Individuals who need help with reading/writing<br />

or those who appear mentally disabled are<br />

given a careful explanation <strong>of</strong> the application<br />

process. All application forms are read to them<br />

and completed for them up to the point <strong>of</strong><br />

obtaining their signature.<br />

• Individuals who appear to be mentally disabled<br />

and eligible to GR are assigned to and see by<br />

an intake Worker, on a priority basis. If the<br />

individual is too disturbed to work with eligibility<br />

staff or requires urgent mental health services,<br />

the individual is referred immediately to colocated<br />

DMH/APS staff.<br />

Refer to GR 41-102 for instructions for<br />

identifying NSA cases.<br />

Refer to GR 41-105 for deferred eligibility<br />

requirements.<br />
