GR 45-115.2 Case Open<br />

in Same District<br />

What needs to be done when an individual<br />

currently receiving GR enters a B&C facility and<br />

the case is in the same district?<br />

When the case is in the same district:<br />

• The district B&C liaison notifies the ES <strong>of</strong> the Unit<br />

tat has the individual’s open case that the<br />

individual is now in a B&C facility.<br />

• The ES immediately transfers the open case to<br />

the B&C Unit/file.<br />

• The case is not terminated.<br />

Refer to WA NO. 40 Case Transfers for<br />

LEADER procedures on case transfers.<br />

• The B&C Liaison schedules a district <strong>of</strong>fice<br />

interview (or home call, if appropriate) within five<br />

workdays <strong>of</strong> receiving the information that the<br />

individual is in the facility.<br />

• The Worker changes the living arrangement on<br />

the Case Individual Summary screen and<br />

processes the B&C case according to policy<br />

contained in this section.<br />

Cases When Individual<br />

Moves From One B&C<br />

to Another<br />

What needs to be done when an approved case<br />

is transferred between districts because the<br />

individual moves from one B&C facility to<br />

another?<br />

The B&C Liaison responsible for handling the B&C<br />

facility (receiving district) notifies the B&C Liaison in<br />

the case-carrying district (sending district) by<br />

telephone or Board & Care Case Transfer Fax that<br />

the individual is now in a B&C placement in the<br />

receiving district’s service area.<br />
