the NSA remains ineligible, the PA 759 is<br />

completed and hand-carried to the DMH/APS<br />

Worker.<br />

• The PA 759 is annotated by DMH/APS and<br />

returned to the worker within two workdays.<br />

Step 2 – Administrative Approval<br />

• Once the PA 759 is returned from the DMH/APS<br />

staff, the original PA 759 and needed supporting<br />

documentation is submitted to the DDD for<br />

confirmation <strong>of</strong> the proposed action.<br />

NOTE: Termination action is only taken when<br />

the DDD agrees that the case should<br />

be terminated.<br />

Step 3 – Allowing a Grace Period For Loss <strong>of</strong><br />

Contact<br />

• A grace period <strong>of</strong> five workdays is allowed<br />

before completing the denial or termination<br />

action when the reason for the adverse action is<br />

“loss <strong>of</strong> contact.”<br />

• Day one <strong>of</strong> the five day grace period begins on<br />

the first workday following the due date for the<br />

requested documents or the appointment date.<br />

• The five day grace period does not include the<br />

two workdays that DMH/APS staff has to return<br />

the completed PA 759.<br />

Verification: The PA 759, with the approval <strong>of</strong> the<br />

co-located DMH/APS staff and the DDD is the<br />

verification needed to terminate/deny an NSA case.<br />

Are there special management responsibilities<br />

for NSA cases?<br />

Yes, there are several areas <strong>of</strong> responsibility:<br />

• Meetings with DMH/APS<br />

At least once a month, the DD and DDD are to<br />

meet with the DMH/APS Worker and his/her<br />

supervisor to discuss the co-location project and<br />

to resolve any identified problems. If districts<br />

share DMH/APS staff, representatives from<br />

each district attend the meetings.
