Completing the<br />

Application<br />

How is the application completed?<br />

The application is completed by the Worker<br />

when the individual is unable to do so due to<br />

illiteracy, language difficulties, or mental/physical<br />

disability.<br />

When the EW completes the application, the<br />

reason the individual was unable to do so is<br />

recorded in Case Comments.<br />

Prior to GR approval, all adults requesting aid<br />

must sign the SAWS 2.<br />

A signature by mark is only acceptable when the<br />

individual is prevented form signing because <strong>of</strong><br />

illiteracy, disease, or mental/physical disability.<br />

A signature by mark (normally an “X”) must be<br />

witnessed by an adult. The adult signing as<br />

witness must indicate his/her address. When<br />

the Worker responsible for the case signs as a<br />

witness, the district address is used.<br />

A signature consisting <strong>of</strong> alphabet characters<br />

different from those used in English (for<br />

example, Japanese, Hebrew, Russian, etc.)<br />

must be witnessed in the same manner as<br />

witnessing a signature by mark.<br />

The witness to such a signature is not required<br />

to understand the foreign language symbols.<br />

The signature <strong>of</strong> the witness is sufficient to<br />

establish that the individual wrote the symbols to<br />

represent his/her signature.<br />

Where there is a court-appointed guardian or<br />

conservator, the application is signed in the<br />

following manner:<br />

“John Smith, ___(guardian/conservator)___ for<br />

the Person and/or Estate <strong>of</strong> Mary Jones.”<br />

Application Interview<br />

What does the application interview consist <strong>of</strong>?<br />

All individuals applying for GR are interviewed to<br />

determine eligibility and the grant amount. Each<br />

adult (18 and over) requesting GR is pre-screened<br />

for potential substance abuse. Each adult or minor<br />

who requests GR is advised <strong>of</strong> rights and
