‣ The notification letter from the insurance<br />

carrier or employer, to the individual that<br />

tells about benefit eligibility, must be<br />

reviewed. The letter notifying the<br />

individual <strong>of</strong> an award, delay, or denial <strong>of</strong><br />

benefits should be mailed to the individual<br />

two weeks after applying for benefits.<br />

‣ If the claim is approved, the notification<br />

letter will contain the start date <strong>of</strong><br />

benefits, weekly amount <strong>of</strong> benefits, and<br />

maximum benefit.<br />

Is any interim verification acceptable?<br />

Yes, the PA 853-2 may be used for interim<br />

documentation <strong>of</strong> Workers’ Compensation benefits<br />

if documents in the individual’s possession do not<br />

provide complete documentation.<br />

The individual submitted verification <strong>of</strong><br />

Workers’ Compensation, what needs to be done<br />

now?<br />

If the verification provided by the individual is<br />

sufficient, the following action, as appropriate, is<br />

taken:<br />

• Refer to Welfare Fraud Prevention and<br />

Investigation Section (WFP&I)<br />

• Enter current income onto LEADER and<br />

LEADER will determine what action is<br />

appropriate (determine and adjust<br />

overpayments, and/or deny/terminate the case).<br />

Information<br />

The verification provided by the individual is not<br />

sufficient verification, what needs to be done?<br />

If the Workers’ Compensation notification letter<br />

does not contain the information specified above, or<br />

if the individual claims no such letter was received,<br />

the employer is contacted using the PA 1778,<br />

Request for Disability Insurance Information, (an out<br />

<strong>of</strong> drawer form) to secure any missing information.<br />
