identify individuals who appear ineligible to GR.<br />

Who is responsible for the screening process?<br />

GR screening may be completed by an intake Task<br />

Force (screeners) or by the intake Worker.<br />

What should be done if there is doubt about an<br />

individual’s eligibility to GR?<br />

If there is any doubt about an individual’s eligibility<br />

to GR, or if an obviously ineligible person wishes to<br />

apply for GR, the application must be accepted.<br />

Following screening, potentially eligible employable<br />

individuals are referred to the Employment<br />

Specialist via the ABP 154, GROW Employment<br />

Specialist Referral, (an out <strong>of</strong> drawer form) for an<br />

Employment Needs Evaluation.<br />

Screening Process<br />

What takes place in the screening process?<br />

As part <strong>of</strong> the screening process, the PA 106,<br />

Would You Like to Register to Vote, (a LEADER<br />

generated form), is given to all individuals. When<br />

the individuals indicate they would like to register, a<br />

Voter Registration form is provided in their primary<br />

language.<br />

What forms are used for GR applications?<br />

The SAWS 1, Application for Cash Aid, Food<br />

Stamps and/or Med-Cal/State-Run County<br />

Medical <strong>Services</strong> Program, is an on-line<br />

LEADER application for GR.<br />

The SAWS 2, Statement <strong>of</strong> Facts Cash Aid,<br />

Food Stamps and/or Medi-Cal/State-Run County<br />

Medical <strong>Services</strong> Program, is printed <strong>of</strong>f<br />

LEADER (after completing the SAWS 1) and<br />

signed by the individual.<br />
