Replacement Card<br />

Are individuals who cannot provide a <strong>Social</strong><br />

Security card or stub required to apply for a new<br />

or replacement card?<br />

Yes, when an individual cannot provide a <strong>Social</strong><br />

security card/stub, they are required to apply for a<br />

new or replacement card with SSA and provided<br />

verification <strong>of</strong> application.<br />

Individuals who have ID and documents for<br />

pro<strong>of</strong> <strong>of</strong> birth/citizenship or alien status<br />

acceptable to SSA are referred for a<br />

new/replacement <strong>Social</strong> Security card at the<br />

time <strong>of</strong> application.<br />

Individuals who do not have ID or documents for<br />

pro<strong>of</strong> or birth/citizenship or alien status<br />

acceptable to SSA re referred after they have<br />

obtained documents that are acceptable to SSA.<br />

Individuals without SSNs are referred to SSA at<br />

the next annual agreement following receipt <strong>of</strong><br />

ID acceptable to SSA.<br />

How is the individual referred to SSA?<br />

The MC 194-LA, <strong>Social</strong> Security Administration<br />

Referral Notice, (a LEADER generated form) is<br />

used to refer individuals to SSA to apply for an<br />

SSN, a replacement <strong>Social</strong> Security card, or to<br />

resolve discrepancies in the event <strong>of</strong> multiple<br />

SSNs.<br />

The yellow copy <strong>of</strong> the MC 194-LA completed by<br />

an SSA representative is acceptable verification<br />

<strong>of</strong> an individual’s compliance with the<br />

requirement to file an application with SSA.<br />

A 30-calendar day FAC for compliance with the<br />

SSA application requirement is set.<br />

A 90-calendar day control from the date <strong>of</strong> the<br />

application at SSA for receipt <strong>of</strong> the <strong>Social</strong><br />

Security card or clarification <strong>of</strong> the SSN as<br />

reported on the MC 194-LA is set.<br />
