GR 44-111.2 –<br />

Verification for Private<br />

Plans<br />

Verification: Viewing the documents specified<br />

below and recording the date the verification is seen<br />

and all pertinent information such as the<br />

award/denial/appeal data in Case Comments.<br />

• The notification letter from the insurance carrier<br />

or former employer to the individual which<br />

explains eligibility must be reviewed. The<br />

notification letter <strong>of</strong> approval <strong>of</strong> benefits is<br />

usually enclosed with the first check to the<br />

individual.<br />

• The benefit notification letter which will contain<br />

the:<br />

‣ Start date <strong>of</strong> benefits<br />

‣ Weekly amount <strong>of</strong> benefits, and<br />

‣ Maximum benefit.<br />

GR 44-111.3 – Interim<br />

Verification<br />

Is any interim verification accepted?<br />

The PA 853-2 may be used for interim<br />

documentation <strong>of</strong> disability benefits if documents in<br />

the individual’s possession do not provide complete<br />

documentation.<br />

GR 44-111.4 – Sufficient<br />

Verification Received<br />

The individual submitted sufficient verification<br />

<strong>of</strong> receipt <strong>of</strong> Private Insurance benefits, what<br />

needs to be done now?<br />

If the documentation provided by the individual is<br />

sufficient, the current income is entered onto<br />

LEADER and LEADER will take action to determine<br />

and adjust overpayments, and/or deny/terminate<br />

the case.<br />

Received<br />

If there is not sufficient verification to determine<br />

Private/Voluntary Plan benefits, what needs to<br />

be done?<br />

If the notification letter does not contain the<br />

information specified above, or if the individual<br />

claims no such letter was received, the employer is
