Assessment Completed<br />

The Assessment Center is responsible for updating<br />

the LEADER system with the assessment results. If<br />

the individual failed to keep/complete the<br />

assessment, the case is denied/terminated and the<br />

appropriate sanction imposed.<br />

What happens when the assessment is<br />

completed?<br />

When the assessment is completed, the<br />

Assessment Center is responsible for entering this<br />

information to LEADER. GR is approved,<br />

regardless <strong>of</strong> the outcome <strong>of</strong> assessment.<br />

Assessment Negative<br />

Can assessment appointments be rescheduled?<br />

The individual may be rescheduled for an<br />

assessment appointment one time; however, aid<br />

cannot be approved until the appointment is kept.<br />

The individual is advised <strong>of</strong> this.<br />

What happens when the assessment outcome is<br />

negative?<br />

When the outcome is negative, the Assessment<br />

Center is responsible for inputting this information to<br />

LEADER and no further action is required.<br />

Assessment Positive<br />

What happens when the individual fails to show<br />

for assessment or refuses to comply?<br />

When the individual fails to show for assessment or<br />

refuses to comply, the Assessment Center is<br />

responsible for inputting this information to LEADER<br />

and non-compliance action is automatically initiated.<br />

What happens when the assessment outcome is<br />

positive?<br />

When the outcome is positive, the Assessment<br />

Center is responsible for referring the individual for<br />

treatment (either residential or outpatient) and<br />

inputting the information to LEADER.<br />

with Assessment<br />

What happens when the assessment outcome is<br />

positive but the individual disagrees with the<br />

assessment?<br />

When the assessment results are contested:
