What happens when the results <strong>of</strong> the presscreening<br />

are positive?<br />

When the results <strong>of</strong> the pres-screening are positive:<br />

An assessment appointment is scheduled on the<br />

Schedule Appointments subsystem, Schedule<br />

GR Sub Assessment Appointment screen.<br />

An ABP 132, Appointment Notice for <strong>General</strong><br />

<strong>Relief</strong> Substance Abuse Recovery Assessment,<br />

is printed on LEADER and given to the<br />

individual.<br />

The individual is advised <strong>of</strong> the assessment<br />

requirement.<br />

Round trip transportation is issued for the<br />

appointment.<br />

Treatment Program<br />

What happens when the results <strong>of</strong> the prescreening<br />

are positive and the individual is<br />

currently in a treatment program?<br />

An assessment appointment is scheduled on the<br />

GR Sub Assessment Appointment screen.<br />

An ABP 132, Appointment Notice for <strong>General</strong><br />

<strong>Relief</strong> Substance Abuse Recovery Assessment,<br />

is printed <strong>of</strong>f LEADER and given to the<br />

individual.<br />

The individual is advised <strong>of</strong> the assessment<br />

requirement to determine whether the current<br />

treatment program is appropriate.<br />

Round trip transportation is issued for the<br />

treatment appointment.<br />

Assessment<br />

Are individuals required to keep the assessment<br />

appointment?<br />

Yes, individuals are required to keep and complete<br />

their assessment appointments as a condition <strong>of</strong><br />

GR eligibility. Failure to keep or complete the<br />

assessment could result in GR denial/termination<br />

and sanction.<br />

the assessment?
