• The gram is forwarded to the B&C Liaison for<br />

faxing to SOS/VPU.<br />

NOTE: The B&C living arrangement cannot be<br />

input to LEADER until SOS/VPU adds the facility<br />

to the listing.<br />

• A 10-day Future Action Control (FAC) is set by<br />

the Worker for the response from SOS/VPU. If<br />

no response is received in ten workdays, the<br />

B&C Liaison contacts SOS/VPU.<br />

• Once the two-way fax response has been<br />

received, indicating the facility has been added to<br />

the list, the case may be processed.<br />

GR 45-105 Placement<br />

by DHS<br />

How is the effective date <strong>of</strong> B&C determined if<br />

the individual is placed in B&C by the<br />

<strong>Department</strong> <strong>of</strong> Health <strong>Services</strong> (DHS)?<br />

• In rare situations, DHS notifies the district that<br />

they have placed an individual in a B&C facility.<br />

In this situation, the application for GR is obtained<br />

as soon as possible, but no later than the date <strong>of</strong><br />

the <strong>of</strong>fice interview or eligibility home call. B&C<br />

may be paid to the facility for up to the seven<br />

calendar days without a GR application. The<br />

effective date <strong>of</strong> aid for B&C placements made by<br />

DHS is the date the individual entered the facility.<br />

• Case Comments are updated with placement<br />

information.<br />

• The B&C vendor is paid the B&C rate for up to<br />

seven calendar days if the individual leaves the<br />

facility before making a GR application. No<br />

payment for personal, incidental, and clothing<br />

allowance is made. The period <strong>of</strong> eligibility is<br />

based on the date the individual entered the B&C<br />

facility through the last day the individual slept at<br />

the B&C facility (not to exceed seven calendar<br />

days).<br />

EXAMPLE:<br />

DHS places an individual in a B&C facility on June 5.<br />

The effective date <strong>of</strong> aid is June 5. The individual<br />

leaves the facility on June 12 before making a GR<br />

application; the facility may receive payment for B&C<br />

from June 5 through June 11. Payment beyond
