• An ABP 320-3, Certification <strong>of</strong> GR B&C Client’s<br />

Signature, (a LEADER generated form) is<br />

required for all individuals in B&C.<br />

• The individual in B&C signs an ABP 320-2,<br />

Vendor Service Order and Invoice Sign-In<br />

Register, (an out <strong>of</strong> drawer form) daily.<br />

The ABP 320-2 is only signed for days that the<br />

individual actually sleeps at the facility.<br />

B&C facilities are not paid for those days that the<br />

Sign-In Register is not signed, except for new<br />

B&C vendors who do not have the forms when<br />

the individual enters the facility. New B&C<br />

vendors are exempt from this policy for seven<br />

calendar days beginning the day after the<br />

SOS/VPU mails the B&C vendor a supply <strong>of</strong> the<br />

Sign-In Registers.<br />

The Sign-In Register cannot be pre-signed.<br />

Management Information and Evaluation (MIE)<br />

monitors the process by reviewing B&C facilities.<br />

Is there a list <strong>of</strong> B&C facilities/vendors?<br />

Yes, the listing can be printed <strong>of</strong>f LEADER.<br />

When is the initial interview scheduled?<br />

The district B&C Liaison schedules a district <strong>of</strong>fice<br />

interview or home call within five workdays after<br />

being notified than an individual entered a B&C<br />

facility. If the individual is not already receiving GR,<br />

the application for GR is usually taken in the district<br />

<strong>of</strong>fice, but in certain circumstances a home call may<br />

be necessary to obtain the GR application. Refer to<br />

the next question for details on home calls.<br />

When are home calls made?<br />

District administrative staff authorize a home call<br />

only when the:
