This Report: Gives You This Information:
Provider Training Report Number of credits received by selected Providers for
completing training courses
Provider Training Due Report Providers that are due to complete a required training
procedure
Provider Weekly Report Number of children in each activity involved in each stage
of the enrollment process
Shots Report Shots that have been given and shots that are due for a
selected range of shots, due dates and family members
Shots/Assessment Labels & Letters
Mailing labels for a range of CYMS enrollees
Mail merge letters for a range of CYMS enrollees
Export file for a range of CYMS enrollees
Report for a range of CYMS enrollees
Special Needs Labels & Letters Export file for CYS enrollees with special needs
Mail merge for CYS enrollees with special needs
Mailing labels for CYS enrollees with special needs
Report of CYS enrollees with special needs
Staff Audit Report Listing of Staff Members who are missing Core
Information data, such as Pay Plan, Pay Grade and
Assignment that is critical to the Annual Report.
Staff Background Check Report Background check information for a selected range of
CYS staff members
Staff Certification Report Status of CYS staff members’ certifications
Staff Credentials/Evaluation Report Lists Credential and Evaluation award and renewal dates
for Staff Members.
Staff Last Evaluation Report Allows you to print a list of Staff Members whose last
evaluation record in CYMS occurred on or before the Last
Evaluation Cutoff Date entered in the selection criteria of
the report. This report also provides a listing of Staff
Members who have never had an evaluation recorded in
CYMS.
Staff Listing CYS staff member setup information
Staff Listing II Allows you to list pre-selected information about your
Staff Members using the same reporting flexibility found
in the Activity Roster. Using selections on the Details
screen, you can define what information you want the
