Manual de Emergencias enGastroenterología y HepatologíaDOLOR ABDOMINAL AGUDO12345Isquemia mesentérica aguda (IAM)La IMA surge como consecuencia del déficit de aporte sanguíneodependiente de la AMS pudiendo afectar, por tanto, alintestino delgado y/o al colon derecho. Puede ser la consecuenciade una interrupción brusca del flujo arterial (embolia,trombosis o insuficiencia mesentérica no oclusiva) o del flujovenoso (trombosis).Embolia de la AMSRepresenta aproximadamente el 50% de los episodios de IMA.Cuando el émbolo se aloja por encima de la bifurcación de laarteria ileocólica se utiliza el término «embolia mayor». Por debajode esta bifurcación o en cualquiera de las ramas distales, sehabla de «embolia menor». Si el émbolo queda impactado muycerca del origen de la AMS (15%) la afectación intestinal puedeser extensa.Trombosis de la AMSRepresenta el 15% de los casos de IMA y afecta a pacientes deedad avanzada con marcada arterioesclerosis. Suele ocurrir enlos 2 cm proximales al origen de una rama arterial importante ysus efectos suelen ser devastadores al afectar a extensas áreasdel intestino, desde el duodeno hasta el colon transverso.Insuficiencia vascular mesentérica no oclusivaEs responsable del 20-30% de los episodios de IMA y surge comoconsecuencia de la vasoconstricción esplácnica debida al efectode sustancias vasoactivas liberadas en respuesta a una situaciónde bajo gasto (shock, arritmias, insuficiencia cardíaca).Trombosis venosa mesentéricaIncluye tres variantes: aguda, subaguda y crónica. Sólo la primerase comporta como una IMA y representa el 10% de loscasos. La trombosis puede ser primaria cuando se debe a untrastorno de coagulación (30%) y secundaria cuando obedecea otros mecanismos, como la pileflebitis (apendicitis, diverticulitiso pancreatitis) o la hipertensión portal.118
