Manual de Emergencias enGastroenterología y HepatologíaENTIDADES EN LAS QUE PREDOMINA EL AUMENTO DE LA ALT.EMERGENCIAS EN HEPATOLOGÍAComo en el caso anterior, los hallazgos de una ecografía, unidos a la presentaciónclínica y los datos de laboratorio (incluyendo marcadores virales)permiten orientar la actitud. Tres cuadros destacan por su relevanciaclínica.12Toxemia gravídica. Aparece en el 2º-3º trimestre y el diagnósticoviene sugerido por la presencia de edemas, HTA y proteinuria (preeclampsia).Cuando se suma la aparición de convulsiones y coma(encefalopatía hipertensiva) se establece el diagnóstico deeclampsia. Las formas más graves se acompañan de afectaciónhepática, siendo común la presencia de dolor en hipocondrio derecho,náuseas y vómitos.Esteatosis aguda del embarazo. Es un proceso grave que apareceen el 3º trimestre del embarazo, con repercusiones graves para lamadre y el hijo. La frecuencia es de un caso entre 7.000 a 16.000embarazos. En el 50% se trata de mujeres jóvenes, obesas y nulíparas.Sus manifestaciones clínicas se resumen en los siguiente puntos:• El inicio es inespecífico con náuseas, vómitos, pérdida de peso,cansancio y dolor en el hemiabdomen superior.• Con frecuencia evoluciona a formas más graves con encefalopatíahepática, edema pulmonar, ascitis, coagulopatía de consumoy fallo multiorgánico con tendencia al sangrado de origen ginecológicoo digestivo.• El examen físico es de poca utilidad:o La ictericia no es muy intensa y no se aprecia hepatomegalia.o La hipotensión es habitual, excepto si coexiste con preeclampsia.• En la analítica destaca una anemia normocítica, leucocitosis ytrombopenia o plaquetas normales.• Las transaminasas y la fosfatasa alcalina están algo aumentadas.• Otros datos son la hipoglucemia y el aumento de urea y creatinina.El tiempo de protrombina y el tiempo parcial de tromboplastinaactivada (TPTA) están alargados y el fibrinógeno disminuído.La ecografía muestra una hiperecogenicidad difusa lo que sugiereun acumulo de grasa, siendo la TAC más fiable. No obstante, ningunade las dos técnicas es suficientemente sensible. La mortalidadfetal es del 23% y la materna del 18%. En general el tratamiento essimilar al HELLP y consiste en poner fin al embarazo. El T.O.H. tiene unpapel muy limitado.366
