Criterios diagnósticos de ascitis refractariaDosis y duración deltratatamiento diureticoAusencia de respuestasRecidiva precoz de la ascitisAparición de complicacionesasociadas al tto diurético.Al menos 1 semanas con espirolactona (400 mg/día) y furosemida(160 mg/día) + dieta hiposódica (90 mmol/día - 2 g).Pérdida media de peso < 0.8 kg durante 4 días y excrección urinariade Na inferior a la ingerida (ver punto 1).Reaparición de la ascitis de un grado 2 o 3 dentro de las 4 semanassiguientes a su deplección.· Encefalopatía en ausencia de otro factor precipitante.· I.R.inducida por diuréticos: incremento de los niveles séricos decreatinina por encima del 100% hasta un valor >2 mg/dL enpacientes con ascitis que responden bien al tratº diurético.· Hiponatremia inducida por diuréticos: descenso de los nivelesplasmáticos de Na > 10 mmol/L hasta alcanzar valores < 125mmol/L.· Hiper o hipokaliemia: cambio en los niveles séricos de potasio > 6mmol/L o < 3 mmol/L, respectivamente, a pesar de medidasapropiadas para normalizar sus niveles.EMERGENCIAS EN HEPATOLOGIATabla II. Criterios para el diagnóstico del Síndrome Hepatorrenal1. Cirrosis con ascitis.2. Niveles de creatinina sérica > 133 µmol/L (1.5 mg/dL).3. Ausencia de mejoría en los niveles de creatinina sérica (? 133 µmol/L) después deal menos 2 días de retirar diuréticos e indicar expansión plasmática con albúmina(dosis recomendada: 1 g/kg por día hsta un máximo de 100 g/día).4. Ausencia de shock5. Ausencia de tratamiento actual o reciente con fármacos nefrótoxicos.6. Ausencia de enfermedad parenquimatosa renal evidenciada por la presencia deproteinuria > 500 mg/día, microhematuria (> 50 hematíes por campo), y/oalteraciones en la ecoestructura del riñón en la ultrasonografía.BIBLIOGRAFÍA1.
