La trombosis suele ocurrir en los 2 cm proximalesal origen de una rama arterial importante y sus efectos suelenser devastadores al afectar a extensas áreas del intestino 1 .No es inhabitual que el enfermo refiera una historia de anginaintestinal en las semanas o meses que preceden al episodiotrombótico.En presencia de peritonitis debe indicarse laparotomía con ladoble finalidad de efectuar revascularización (no es suficiente lasimple trombectomía, sino que debe asociarse una derivaciónmediante reimplantación de la arteria ocluida, bypass o injerto)y exéresis del tejido necrótico, siguiendo los mismos principios:infusión intraarterial de papaverina, antes, durante y después dela intervención, no resecar intestino antes de repermeabilizar elvaso y explorar el campo operatorio 12-24 horas después. Lospacientes que sobreviven deben ser sometidos a descoagulaciónsistémica a partir de las 48-72 h.En ausencia de peritonitis es importante verificar medianteangiografía la presencia o ausencia de circulación colateral. Enlos casos que muestran una buena circulación colateral y unarepleción suficiente del lecho de la AMS hay que dudar que delos síntomas tengan su origen en una TMA. Es probable que elpaciente tuviese lesiones ateromatosas en distintos puntos, peroel grado de oclusión de la arteria no justifique el cuadro actual.En tales casos es prudente observar al paciente y/o pensar enun diagnóstico alternativo.Si el riesgo quirúrgico es elevado y no existe peritonitis puedeplantearse la perfusión de agentes trombolíticos (uroquinasa:bolus de 2 x 10 5 U intraarteriales seguidos de perfusión de 60.000-200.000 U/h) o un procedimiento de revascularización no quirúrgico(angioplastia con o sin stent).1301Desde el duodeno hasta el colon transverso.
