Manual de Emergencias enGastroenterología y Hepatología1La cirugía de revascularización ha sido durante décadas elmétodo más utilizado por la mayoría de los pacientes con IMC.Los procedimientos incluyen el bypass mediante injerto realizadopor vía retrógrada o anterógrada, la reimplantación de laarteria mesentérica superior (AMS) en la aorta abdominal y laendarterectomía mesentérica transarterial y transaórtica. Losresultados varían en función del tipo de procedimiento utilizado,el número de vasos revascularizados y la posibilidad de llevar acabo otras intervenciones concomitantes, como la reconstrucciónaórtica. Así, el bypass único de la AMS puede ser tan eficazcomo el bypass de múltiples vasos, en términos de alivio sintomático,soslayando alguna de las morbilidades asociadas alprocedimiento de reconstrucción múltiple. Una evaluaciónretrospectiva de todas las series comunicadas, incluyendo untotal de 880 pacientes, permite concluir que la mortalidad delprocedimiento se sitúa por debajo del 10%, el éxito terapéuticoes del 90% y la tasa de recidiva inferior al 10% (tabla I).DOLOR ABDOMINAL AGUDO2La angioplastia percutánea transluminal (AMPT) ha sido utilizadapara repermeabilizar los vasos ocluidos. La tasa de éxito evaluadaen términos de alivio síntomático oscila entre el 63 y el100%, con una baja mortalidad. Sin embargo, la AMPT conllevauna mayor tasa de recidiva sintomática, que oscila entre el 0 yel 63% según las series (tabla II). Este riesgo disminuye si se colocaun stent, una vez efectuada la angioplastia. En el futuro, laAMPT con stent puede llegar a ser el tratamiento estándar enestos enfermos. Entretanto, la opción terapéutica debe definirseen función del riesgo quirúrgico. Si éste es razonablemente bajo,se recomienda la revascularización quirúrgica. Cuando aquél esapreciable, la mejor opción es la AMPT, preferiblemente con unstent.144
