Manual de Emergencias enGastroenterología y HepatologíaTRATAMIENTO DE LAS COMPLICACIONES PARENQUIMATOSASEN EL PERÍODO PRECOZ POSTRASPLANTEEMERGENCIAS EN HEPATOLOGIA123456El tratamiento del rechazo agudo o mejor llamado rechazocelular depende fundamentalmente del grado de afectación(Tabla 2).El grado leve, caracterizado por inflamación y alteración de losconductos biliares en menos de la mitad de los espacios porta,no precisa tratamiento y es suficiente con realizar un seguimiento.El grado moderado, que afecta a más de la mitad de los espaciosporta, se puede tratar con ciclos de esteroides a dosisaltas. Sin embargo, se ha observado que la utilización de corticoidesa dosis altas en pacientes con infección por el VHC condicionauna peor evolución y una mayor tasa de pérdida delinjerto. Por esta razón, en los pacientes trasplantados con VHCes preferible incrementar la inmunosupresión basal o inclusoañadir nuevos fármacos inmunosupresores, con el fin de evitar,en lo posible, el tratamiento con bolos de corticoides.El rechazo agudo grave, se caracteriza por la presencia denecrosis centrolobulillar y precisa tratamiento con corticoides adosis altas. El tratamiento más habitual es el de bolos de metilprednisolona(0.5-1 gr al día) durante tres días.Cuando no hay respuesta al tratamiento con corticoides hablamosde rechazo corticorresistente. En el rechazo corticorresistentese puede cambiar el inmunosupresor de base, añadirotro fármaco inmunosupresor (ej: micofenolato), o utilizar anticuerposanti CD25 como el daclizumab o el basiliximab. Hoydía, en esta situación ya no se utilizan los anticuerpos monoclonalesOKT3 debido a sus efectos secundarios.La hepatitis aguda debida a la reinfección por VHC generalmenteno se trata debido al riesgo que comporta el tratamientoantiviral en un período tan precoz. Se deben realizar controlesanalíticos y un estudio anatomopatológico a los 6-12 mesesdel trasplante, optando por el tratamiento antiviral en funcióndel grado de afectación.360
