Manual de Emergencias enGastroenterología y HepatologíaCONSECUENCIAS DE LA HTP EN LA CIRCULACIÓN ESPLÁCNICAEMERGENCIAS EN HEPATOLOGÍA296El aumento de la presión portal origina cambios en la circulaciónvenosa y arterial esplácnica.123Circulación venosa esplácnica: el incremento de las resistenciasvasculares en el territorio sinusoidal y el aumento consiguiente de lapresión portal promueve la apertura de colaterales portosistémicasque desvían una proporción significativa del flujo esplácnico haciala circulación sistémica evitando el filtro hepático. A ello se une unfenómeno de neoangiogenesis que da lugar a la formación denuevos vasos colaterales. Ello comporta múltiples consecuenciasrelevantes, entre las que destaca la formación de varices esofágicas;su rotura y la hemorragia consiguiente representan la consecuenciamás grave de la HTP. Al componente mecánico o estructural,se suma otro componente dinámico o funcional, dependientede la contracción activa de elementos contráctiles en el tejidohepático. Estos incluyen, no solamente el músculo liso vascular delas vénulas portales y terminales hepáticas, sino también las célulasestrelladas perisinusoidales (células de Ito) y los abundantes miofibroblastoslocalizados en el tejido fibroso.Circulación arterial esplácnica: en el enfermo cirrótico con HTP seha comprobado la existencia de vasodilatación del territorio arterialesplácnico. Esta vasodilatación aumenta la presión capilarintestinal, el flujo linfático y el aflujo de sangre al territorio portal. Estaúltima contribuye a su vez a aumentar la HTP, de acuerdo con laley de Ohms 1 .Los mecanismos por los que la HTP induce el estado de vasodilataciónesplácnica están relacionados con la acción de diferentessustancias vasoactivas, de las que el ON parece desempeñar elpapel más importante (ver Algoritmo 3).1Ley de Ohms: la diferencia de presión entre los dos extremos de unvaso depende de dos factores: (1) el flujo que circula a través del vasoy (2) las resistencias que oponen las paredes del vaso a este flujo. Enfases avanzadas de la cirrosis, los dos componentes de esta ecuaciónestán alterados, dado que al aumento inicial de las resistencias vascularesen el territorio sinusoidal se suma el hiperaflujo venoso portal secundarioa la vasodilatación del territorio arterial esplácnico.
